The situation

What has happened

The General Manager has asked for a quarterly report on injuries, near misses and use of force for the leadership meeting on 6 October. The Incident and Use of Force Register holds the quarter's entries, and the Hazard and Risk Register lists open extreme, high and medium risks, including the Unit 4 assault risk that the staggered unlock was meant to control from 1 September. Two officers have lodged workers compensation claims for psychological injury this quarter. The Custodial Operations Manager is sceptical about the staggered unlock because it costs overtime, and the report will be read by the contract manager.

Your brief. Analyse the quarter's incident and hazard data, evaluate whether current controls are working, and write a clear trend report with recommendations, owners and dates that the General Manager can table at the leadership meeting and share with the contract manager.

What to look for

Evidence guide

The analysis must be accurate against the register data and must be honest about small numbers and short periods when evaluating the unlock control. Recommendations must be specific, owned and dated, and must address the psychological injury trend. Look for evidence of consultation with both managers and a report written for a contract manager audience, not internal shorthand.
