What has happened

Dean Callaghan, RC-118204, was sentenced on 1 September 2026 to three years and six months. His risk and needs assessment rates him medium risk of reoffending with criminogenic needs in alcohol use and anger management, a history of unstable housing and a reading level that makes long written material difficult. He has a partner and a young child in Cessnock and wants to keep contact. The alcohol and other drug program has a waiting list until November. His case plan is due by 29 September and the Offender Services Manager signs off on Thursday. He is on Unit 3 and can be interviewed in the unit interview room this afternoon.

Goals must map to the identified criminogenic needs and be measurable within the sentence. Look for the responsivity factor being handled in the interview plan (plain language, verbal explanation), the waiting list managed with an interim step, the offender's own view recorded honestly, a reintegration section with dates and a correctly updated system record. The sign-off note must link plan to risk.
