Purpose. Case managers use this template to record the risk and needs assessment outcome, the agreed goals and interventions and the review schedule for each sentenced offender.

1.Purpose of the case plan

The case plan is the working agreement between the offender, the case manager and the centre. It turns the risk and needs assessment into concrete goals, programs, education, work and reintegration steps. A plan must be completed within 28 days of sentence and reviewed at least every six months.

2.Assessment summary

Summarise the outcome of the risk and needs assessment, including the overall risk of reoffending, the criminogenic needs identified and any responsivity factors such as literacy, cognitive impairment, mental health, culture or language that affect how interventions must be delivered.

3.Goals and interventions

Record each goal, the program or intervention that addresses it, who delivers it, when it starts and how progress will be measured. Goals must be specific and achievable within the sentence. Where a program has a waiting list, record the referral date.

4.Offender participation

Record the offender's own view of their needs and goals and their agreement to the plan. Where the offender does not agree, record the reason and the case manager's response. The plan must be explained in plain language and, where required, through an interpreter.

5.Reintegration

From the mid-point of the sentence, the plan must address accommodation, identification, income, family contact, health continuity and community supports on release. Referrals to community agencies must be recorded with dates.

6.Review

Set the next review date. At review, record progress against each goal, any new risk information and changes to the plan. Sign and date each review and update the Offender Case Management system.
