Shift Briefing and Handover Template

Purpose. Senior Officers use this template to deliver the start-of-shift briefing and to hand over the unit to the incoming shift with every current risk, task and count recorded.
1.Purpose
The handover is the single point at which the outgoing shift transfers responsibility for the unit to the incoming shift. Anything not recorded on this template is treated as not handed over. The template is completed on every unit at every shift change and retained for two years.
2.Count and population
Record the count at handover, the number of prisoners out of the unit and where they are, and confirm the count matches the central monitoring station figure. A discrepancy must be resolved before the outgoing shift leaves.
3.Risks and alerts
List every prisoner on observations, on protection, on medical alert, on a separation order or subject to an intelligence alert, with the required action for the incoming shift. Include any staff safety alert.
4.Incidents and outstanding tasks
Summarise incidents during the shift with register entry numbers, searches conducted and finds, maintenance faults reported and any task the incoming shift must complete such as escorts, appointments and program movements.
5.Keys, equipment and tally
Confirm the tally of keys, radios, restraints and tools is complete. Record any faulty equipment and the action taken.
6.Sign off
The outgoing and incoming Senior Officers sign and record the time. The briefing must be delivered verbally to all incoming officers using this record before posts are taken.
