Incident Report Form

Purpose. Every officer involved in or witnessing an incident completes this form before leaving the centre. It is attached to the register entry and is a legal record.
1.When to use this form
Use this form for every Category 1, 2 or 3 incident, every use of force, every injury and every near miss. Complete one form per officer. Do not complete a joint report with another officer.
2.How to write the report
Write in the first person, in chronological order and in plain language. State what you saw, heard and did, and the times. Do not include opinion about other officers or guess at facts you did not observe. Name every person present.
3.Use of force
If you used force, describe the warning given, the technique used, the part of the body it was applied to, how long it lasted and why it was necessary. State whether medical assessment was offered and the outcome.
4.Evidence
List any footage, photographs, items or documents that relate to the incident and where they are held. Record the register entry number once it has been created.
5.Submission
Sign and date the form and hand it to the Senior Officer before leaving the centre. The Senior Officer will check it is complete and attach it to the Incident and Use of Force Register entry.
