The situation

What has happened

During afternoon unlock a prisoner in the Unit 3 dayroom refuses a direction to return to his cell, raises his voice and advances on your colleague. You call the incident on the radio, the Senior Officer responds, and you and your colleague apply an approved escort hold and walk the prisoner to his cell. He is offered a medical assessment and accepts. The Senior Officer has created register entry INC-2026-0416 and it shows Reports received, but your Incident Report Form is not yet in and the General Manager reviews every use of force within five working days.

Your brief. Complete your part of the incident response under the Incident Response and Reporting Procedure and the Use of Force Policy: write a first-person Incident Report Form, confirm evidence has been preserved, make sure the welfare steps are recorded and move the register entry to Under review with everything attached.

What to look for

Evidence guide

The report must be first person, chronological, factual and free of opinion about the other officer, and must justify the force as necessary and proportionate with reference to the policy. Check the student identifies the two-hour and five-day timeframes, records evidence and medical outcome, and hands over the prisoner's status. Debrief notes must show reflection, not blame.

The student's evidence summary lists every record they created or changed in the systems named above, their notes and the tasks they ticked. Verify it against the deliverables and your own assessment tool.
